**Job overview**:

- To provide clinical and operational leadership to the Community Treatment Room & Leg Ulcer service.
- To ensure the safe and effective delivery of Adult Community Nursing.
- To provide support and guidance to colleagues to assure that work is carried out in a multi-disciplinary way to promote the needs of clients and patients. In addition the post holder will support the Senior Manager and Operational Manager, to ensure that governance systems and processes are adhered to and delivered within the area of responsibility of the senior practitioner.

**Main duties of the job**:

- Undertake holistic assessments and support staff with those patients who present with complex nursing needs and plan care according in conjunction with the Band 6 Nurse caseload holder and review/evaluate this care as needed.
- Demonstrate and deliver skilled nursing care to the identified caseload of patients utilising best practice and evidence based practice when working with staff. This will be to a minority of patients and only very complex care patients.
- Advise patients, carers, staff, other professionals, and third sector organisations with regards to area of expertise.
- Advise and make referrals, according to need following discussions with patients, carers and caseload managers to other statutory and voluntary agencies.
- Take appropriate action to deal with emergency situations this may be with staffing levels or patient care

**Detailed job description and main responsibilities**:

- Take the lead on Governance for the cluster with regards to nursing practice.
- Assess and work with individual members of staff with regards to assessing competencies and developing staff.
- Develop action plans for individual staff members or teams where there are areas of concern in conjunction with the Senior Manager
- Monitor and sign off supportive action plans when completed.
- Share learning from incidents and disseminate to staff.
- Participate in the wider Pennine Care Trust professional Adult Community Nursing agenda and attend meetings as necessary.
- Providing peer support/ clinical and management supervision with other Senior Practitioners as required

**Essential criteria**:

- Community Specialist Practitioner Qualification (SPQ)
- Degree / diploma in nursing studies (or equivalent nursing qualification and experience

**Desirable criteria**:

- Independent prescriber (V300) or Nurse prescriber willingness to undertake V300 as appropriate

**Knowledge, Training & Experience**:
**Essential criteria**:

- Significant Community experience
- Able to demonstrate clinical practice at a senior level
- Experience of managing a team
- Proven clinical leadership
- Awareness of recent government policies and directives and the implications for community service
- Knowledge of relevant governance systems and their influence on current practice

**Desirable criteria**:

- Management qualification
- ENB 998 or other mentor qualification or equivalent such as Practice Teacher Award
