**Key Responsibilities**:

- Provide high-quality care and support to adults with disabilities and complex needs
- Support individuals with their daily living (inclusive of outings)
- Develop and maintain positive relationships with service users, their families, and other professionals
- Keep accurate and up-to-date records of care and support provided
- Follow all policies and procedures of the organisation
- Communicate effectively with the agency and the clients to ensure smooth and effective service delivery

**Key Requirements**:

- Previous experience in a similar role is essential (3 months minimum
- Passionate about providing high-quality care and support to adults with disabilities and complex needs
- Excellent communication skills, both written and verbal
- Ability to work effectively in a team and independently
- Ability to work flexible hours including weekends and evenings
- A clean criminal record check and vulnerable sector screening
