Our Care Home Liaison Team is a multi-disciplinary team based a Scorton Medical Centre, Scorton, Richmondshire and provides support to our aligned Care Homes, our proactive frailty support and acute visiting service to housebound patients. You will provide care for Richmondshire's care homes and supported living populations, from initial assessment of a new resident, review of residents who have recently been discharged from hospital, review of stable residents and those who are deteriorating and approaching end of life care. You will be able to demonstrate critical thinking in the clinical decision-making process, including assessment and diagnostic skills, leading to the delivery of safe and appropriate care for all patients. Working collaboratively with the care home visiting team to meet the needs of the patients whilst supporting the delivery of policy and procedures.

You will provide a holistic and clinical service, with support from GPs and other team members as required, implementing agreed management plans. You will review and act appropriately on blood results and referrals/correspondence regarding patients. The post holder will work with the wider PCN team to ensure that all aspects of the Primary Care Network Contract Directed Enhanced Service and its associated service specifications, relevant to this post, are realised. The role is pivotal to improving quality of care for patients and operational efficiencies within primary care and therefore requires motivation and passion to deliver excellent service within general practice.

Key responsibilities Responsibilities may include but not be limited to the following: You will have an advanced practice qualification and be a non-medical prescriber Plan and manage complete episodes of care, working in partnership with other members of the multi-disciplinary PCN team, delegating and referring as appropriate, to optimise health outcomes for patients Able to use an advanced level of professional accountability, autonomy and judgement for seeing, treating and assessing patients who present with a broad range of situations and conditions Work autonomously, clinically examining and assessing patients and initiating any appropriate action whilst taking responsibility for managing practice whilst using an advanced level of professional accountability, autonomy and judgement for highly complex decision making across a broad range of situations Have a level of understanding of PCN and NHS values, policies and procedures Provide support to Care Home staff, which may include delivering training where required or supporting Care Home pilots and support the other members of the Care Home Liaison Team Confidentiality In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters.
