Senior Support Worker

Job Purpose:

- To take a lead role in supporting Service Users in their own homes and in the community through person centered approaches in line with their individual support plan
- To provide flexible and supportive services to maximise Service User independence
- To work within the context of Select Support Partnerships policies and procedures
Main Responsibilities and Duties General
1. To work in partnership with Service Users, their families and representatives to implement all aspects of their support plans.
2. To encourage Service Users to have a proactive approach to their lives and to maximise their inclusion and participation in the community according to their needs, wishes and preferences.
3. To promote equality and diversity within the service and the wider community.
4. To maintain confidentiality at all times.
5. To support Service Users to access a range of opportunities and experiences either from their own home or the wider community.
6. To support Service Users to monitor their health and wellbeing.
7. To attend team meetings and supervision meetings and contribute to the promotion of good practice across the service.
8. To undertake all statutory training and comply with legislation requirements.
9. To support Service User’s to reach their full potential and to assist them to meet their goals.
10. To uphold and maintain professional boundaries.
11. To facilitate communication and help Service Users to express their needs, feelings and wishes.
Service User Support
To participate in the full range of support duties including:
12. Supporting Service Users with all aspects of their agreed support plan.
13. Where required and appropriate, support Service Users with all aspects of their personal care needs and ensuring the individuals rights to privacy and dignity are respected.
14. Support Service Users to manage their medication, including administration where appropriate and to support individuals to have an understanding of why they take their medication.
15. Support Service Users to develop skills to enable them to manage their daily lives and to promote their independence.
16. Supporting and encouraging all domestic duties (washing, cleaning, cooking etc.)
17. Support Service Users to manage their finances as required within their support plan.
18. Support Service Users to access paid or unpaid employment / voluntary work, education and leisure opportunities.
19. Support Service Users to deal efficiently with all post and correspondence.
20. Support and accompany Service Users on holidays, outings, trip, visits etc. as required.
21. Facilitate and support meaningful day activities including attending college and work placements etc.
22. Support and promote the development and maintenance of independent living skills.
23. To help ensure that the Service Users health needs are met including support liaising with health professionals, visiting health services and ensuring the implementation of appropriate medication procedures.
24. Assist Service Users to develop and maintain relationships with family and friends.
25. Support Service Users to achieve planned goals and personal outcomes.
26. Support Service Users to become active and valued members of the community.
27. Respect Service Users privacy, property and choices.
28. Actively listen to Service User views, decisions and choices.
29. Inform your manager of any concerns related to the health and wellbeing of the Service User.
30. Support Service Users to make complaints and report any tenancy concerns, problems or hazards.
Recording, Reporting and Administration
31. To complete the daily diaries and any other relevant paperwork during each shift.
32. To accurately complete the MAR sheet in line with the Administration of Medication Policy.
33. To maintain complete and accurate records including Service Users personal information and financial records.
34. To provide regular written or verbal reports to the Team Manager on the progress of individual Service Users.
35. To ensure all Service User appointments are recorded in the house diary and within daily records and HAPS and inform the Team Manager as appropriate.
36. To read the communication book, daily diaries and any other relevant paperwork at the beginning of each shift.

**Salary**: £11.92 per hour

Licence/Certification:
