In the Community Division we have an exciting opportunity for a Discharge Coordinator to join our Community Unit. The Community Unit is based on the St Mary's Hospital.

The Community Unit delivers Rehab in a safe, high-quality care and ensures a positive patient experience within an enabling environment where patients participate in a Rehab programme.

**The Discharge Coordinator will need to demonstrate skills in how to best achieve a safe and timely admission and discharge. The successful applicant will be highly organised and in often challenging situations, they will need to maintain excellent interpersonal and documentation skills and feel confident working as part of a multi-disciplinary team.**
