We're looking for a Deputy Care Coordinator to join our Wakefield team.

If you're a Care Assistant with additional responsibilities such as rota planning or spot checks or you're currently a Deputy Care Coordinator or Care Coordinator, this is the perfect career opportunity for you.
- You have 6 months care experience in the last 3 years, ideally in domiciliary care (home care).
- You can sign up with us to start your level 3 NVQ once you start your employment.
- Ideally have PASS and CP knowledge (desirable, but not essential).
- You can commit to working every other weekend.

**Role and Responsibilities**:

- Allocation of care workers to customer visits by using effective rostering, ensuring continuity of care and taking into account locations, skills required and customer preferences.
- Take responsibility for new referrals from private customers and social services teams, ensuring full information is received and appropriate care planning is undertaken. Assist Branch Manager with identification of further business opportunities outside of current client base.
- Liaise as necessary with partner professionals and organisations (e.g. social workers, GPS, nurses) in respect of developing and delivering holistic, person-centred packages of care
- Dealing with queries from clients and families in a calm and professional manner.
- Organising and supporting with Staff Supervisions and Appraisals
- Attending and participating in Staff team meetings
- Organising Field Care Supervisors work load
- Any other tasks required by the Deputy Manager which she/he deems necessary for the needs of the business.
- On call duties on rota system shared with other senior staff members in the team
- Delivering care in emergency situations
- Weekend work on a rota basis.
