An exciting opportunity has arisen for a Health and Wellbeing Coach to join our Health and Wellbeing Teams in Durham Coast and North Easington PCNs.

In your role as a Health & Wellbeing Coach, you will work as part of the Integrated Personalised Care Teams, alongside SPLWs and Care Co-ordinators. The posts are hosted by one practice but will support patients across all of the practices within the PCNs.

We aim to provide person-centred care, promote health and independence, including use of care navigation to other professionals where appropriate.
