Hounslow Community Neuro Rehab Team have a current vacancy for a part-time Occupational Therapy Rehabilitation Assistant.

**Vacancy**: 0.5WTE - 18.75 hours

This post holder will work across the Neuro Rehabilitation and Long Term condition management pathways within Hounslow CNRT. The post holder will work closely with the Occupational Therapists, to support the provision of high quality therapy and rehabilitation to patients with a variety of neurological conditions.

This job will allow the post holder to gain experience working with other members of the MDT, as well as developing specialised neurorehabilitation skills within community and clinic settings.

Provide high quality assessment and treatment to adults referred to the Community Neuro Rehabilitation Service.

Deliver an individualised treatment programmes to the adult/elderly clients referred to the Community Neuro Rehabilitation Service as directed by lead therapists.

Work closely with the multi-agency team to meet the needs of the clients/patients and enable them to receive a comprehensive service.

Complete assessments and retain case holding status for clients (under supervision) where the client requires progression of clearly stated rehabilitation goals handed over by a referring therapist where recovery is predictable, and protocol based.
