We're urgently hiring a dedicated Care Coordinator to ensure our clients receive the care they need and make an immediate impact

What is a Care Coordinator?

You'll be responsible for coordinating and organising care plans, liaising with healthcare professionals and our care professionals, and making sure our clients receive the highest standard of personalised care.

Key Responsibilities
- Conduct service reviews and lead quality assurance meetings.
- Perform daily audits, promptly addressing concerns.
- Run incident reports, investigate, and escalate issues.
- Provide immediate support to Care Professionals with visits and assessments.
- Maintain accurate records using Home Instead software.
- Build positive and trusting relationships with clients, their families, and care professionals.

**Qualifications**:

- Level 3 NVQ in Health and Social Care or equivalent or willing to work towards it.
- Proven experience in the care sector.
- Passion for delivering urgently high-quality care.
- Excellent communication and rapport-building skills.
- Confidence in using care management technology.
