This is an opportunity to join the energetic, innovative transfer of care hub team.It is an exciting time for complex discharge with many new pathways and initiatives being introduced with partnerships with external agencies being forged by the transfer of care team.

You will provide expertise to support safe, effective and timely discharge working directly with internal and external multidisciplinary teams as required.

This post will offer you the opportunity to develop and flourish whilst learning about the operational management of the Trust. You will be part of a developing team that strives to ensure all patients are given equal opportunity to return home.

You'll need to be enthusiastic and have a strong sense of purpose and good diplomatic skills in engaging with staff, patients and relatives. However you'll also need to be tenacious, confident, calm and organised.
- Responsible for supporting all aspects associated with the facilitation of discharging patients from Ipswich Hospital and the community hospitals, working with the relevant clinical specialties of East Suffolk & North East Essex Foundation Trust to agreed quality standards and within the resources and income base available, to support the Trust’s development as a clinically led organisation.
- To work collaboratively within the Transfer of Care hub as part of the multidisciplinary team.
- Is responsible for supporting care homes and liaising with all care home providers focusing on ‘discharge follow up calls’.
- Is responsible for having a good understanding of the national policy and guidance relating to the safe discharge of patients.
- To provide support to the senior nurses within the hub with challenges to ward pressures to ensure high quality individualised discharge planning for patients and relatives/carers.
- To support the maintenance of the red day tracker ensuring timely data input and provision of daily information regarding discharges, for use at daily planning meetings.
- To support the Transfer of Care hub with daily data collection and high quality administration.
- To triage referrals in the Transfer of Care Hub in a timely manner.
- To work alongside ‘The Nurse in Charge’ in The Transfer of Care Hub providing an operational role.
