Vascular Mdt Coordinator

An opportunity has arisen for you to join as an MDT Coordinator for the Vascular Services team at York Teaching Hospitals NHS Foundation Trust.
The post holder is responsible for ensuring the effective coordination and provision of accurate, complete and timely data in order to meet national requirements for waiting times and audits. Analysis and reporting of data in order to monitor the service against objectives required. To provide effective management of multi-disciplinary (MDT) meetings, working in close partnership with the Clinical Lead, Lead Vascular Nurse Specialist and extended team members.
Liaise with clinicians and administration staff across departments within the Trust and outside units of a daily basis to develop the MDT agenda.
Liaise with clinicians in order to define the appropriate criteria for audit projections and produce data collection forms.
To liaise with information services with regards to identified problems in data collection and ongoing development of the data collection systems and databases.
To maintain confidentiality at all times with regard to patient information.
Develop, foster and maintain effective relationships/partnerships, both internal and external to the Trust.
Maintains up to date knowledge of quality measures and data definitions in relation to waiting times and audit data.
Be able to collate, process and develop routine data to produce timely and accurate information of patient pathways for the Trust.
Have the ability to analyse and interpret data to produce meaningful management information.
Be able to work to tight deadlines and deal with ad hoc requests for information.
Be able to use databases and spreadsheets to an advanced level in order to undertake analysis and reporting.
Be able to understand medical terminology in relation to Vascular Services and undertake training as required.
Be able to collate, coordinate and implement patient information in preparation for effective management of MDT meetings.
Use knowledge and experience of data to inform/advise colleagues regarding data collection.
