The Intensive Outreach Case Management Service IOCMS are looking for an experienced mental health practitioner to undertake expert work surrounding discharge and community pathways on behalf of young people who are within tier 4 services.

The role forms part of the WMCPC approach to reducing dependency within tier 4 services. This specific job will require the appointed professional to support the current providers thinking of alternate clinical pathways and resolve “system blocks”.

Your expertise as a clinician will support the development of integrated health and social care lead discharge plans through a systemic lens. The focus of these will come at a point teams are considering escalation in restrictions so communication skills and knowledge around the Mental health act and children’s act are paramount to achieving successful interventions.

There may be significant periods where the post holder is required to work alone and be autonomous in decision making in clinical matters. This will involve negotiation with other mental health professionals at all levels.

The post holder will hold a high level of skill and knowledge in inpatient care and discharge pathways. Understand the clinical management and community interventions for complex presentations and support networks to conceptualize effective and meaningful discharge pathways.

The duties will include to anticipate, co-ordinate and join up health and social care professionals on behalf of patients displaying high risk behaviors which lead to unplanned escalation in restrictive inpatient care.

To act in a consultative capacity to support meaningful delivery around parallel planning when a young person’s risk behaviors are reducing MDT’s vision for the care plan.

The use of statutory legislation and guidance under the Mental health act and children’s act in order to fulfill CPA care plans.

Working as an autonomous clinical leader with the ability to traverse inpatient MDT’s and influence thinking. This is in the context of systemic barriers when the only option for inpatient teams requiring escalation in restrictions to manage risk.

Abstract and dynamic evidence-based planning support and advice to services whose planning involves.

Effective referral and coordination of safe and appropriate community pathways.
