The role of the Twilight Health Care Assistant is to provide care that meets the holistic needs of individual patients and their carers/families, including physical, emotional, spiritual, practical and pre and post bereavement needs. If you can pro-actively contribute to the delivery of high-quality safe evidence-based care, then this role is for you.
- **Hours**: part time 18:00pm - 12:00 midnight
- **Salary**: £20,520 - £23,450 per annum (pro rata)
- **Closing date**: 3 October 2023 at 12 noon

You will therefore need to demonstrate your abilities in the following areas:

- Proactively contribute to the delivery of high-quality safe evidence-based care.
- Deliver high quality Specialist Palliative and End of Life care as part of the In
- Patient Unitmultidisciplinary team.
- Under the direction of a multi skilled team, support the assessment, planning and evaluation of individualised patient centred care, ensuring a holistic approach that is sensitive and responsive to the patients' ever-changing needs.
- To support the smooth running of a shift, ensuring staff and patients are safe.
- To support the management of calls (call handling service) for all of our community services.
- As a member of the wider organisation, demonstrate delivery of the agreed vision and values.

- limiting illness and those who care for them, to live as well as possible by providing choice and ease access to a wide range of palliative care and bereavement services across West Hertfordshire and Buckinghamshire.

We provide care and support for adults and children diagnosed with cancer and other life-limiting illness, and their families, visiting day and night. Its important work and, to make it all happen, we need talented and passionate people.

If successful, you'll join on the first working Monday of the month so we can best support you with our structured induction programme.
