**About us**
Moving On Durham are the leading independent provider of supported accommodation for young people aged 17-25 across County Durham. This post is a opportunity reflecting our ambition and the need for us to provide specialist support services supporting and mentoring Young Men in our accomodation.
Our aim is to break the cycle of youth homelessness in County Durham. We work with young people individually, using strength based methods to address their challenges. We enable young people’s transition from a place of chaotic housing need to a position of security, knowledge and confidence. We are not a housing association. We don’t own property or place unnecessary focus on the acquisition and management of housing stock. This allows us to relentlessly focus on the needs of our young people, placing their development at the front and centre of our business model.
Our support team is made up of specialist youth workers, supported by a professional housing team. This blend of people allows us to put the development of life-skills first, with specialist input from housing professionals to help young people sustain their tenancies.
Moving On was first established as a project called Durham Night Stop in the late 1980’s. The project was a partnership between Durham Churches Group and Durham City Youth Project who were concerned about the poor provision of homeless support and supported lodgings in the City. It operated as a project of the Durham City Youth Project, before becoming formally constituted as Moving on Durham in 1996.
Our vision is that young people are able to live independently and sustainably in the community. We aim to achieve the vision through the delivery of all our charitable programmes and in particular, through the achievement of our core outcomes:
- Young people develop good tenancy management skills
- Young people develop an understanding of their place/role in society
- Young people improve their physical and mental health
We achieve impact through the delivery of our main programmes which include homelessness prevention, property services, housing support and support for young people to move to sustainable independent living in the community.
**Moving On**:
Provides information, advice and guidance to young people across County Durham area who are in housing need.
Helps young people to access and sustain appropriate, stable and secure accommodation.
Offers housing support to help young people with their independent living skills in order to help them succeed in their tenancies.
Coordinates with local agencies and young people themselves to address the needs of homeless young people and those at risk of becoming homeless.
**Aims of the post**:
The aim of the post is to support young people in sustaining their tenancies through the following:
Actively seek to empower young people to gain and maintain control over all aspects of their lives
Recognise young people's strengths and skills and seek to encourage their development
Provide needs led support, advice and practical guidance to young people who have their own tenancies.
Undertake support needs assessments, develop mutually agreed support plans with young people and provide tenancy support services.
Link young people to other support services as appropriate - internally and externally.
Develop and maintain close working relationships with relevant individuals and organisations working with young people in the Durham area.
Work with the other Moving On staff and partner agencies to further the aims of the charity.
Main duties:
1. Undertake individual needs assessments and implement support plans, largely through direct work with young people in their own homes and ensure they effectively access other specialised services if appropriate.
2. Work with young men for 6 - 12 months to develop independent living skills.
3. Provide responsive and flexible support as required by the individual (which may include outside of office hours if for instance there are emergency issues which need dealing with).
4. Implement effective withdrawal of support at the appropriate time.
5. Regularly update and participate in the monitoring and evaluating of the scheme including use of the Inform database.
6. Contribute to written reports as required by the Board of Trustees/Chief Executive.
7. Work within and maintain records in accordance with the organisation’s policies and procedures particularly around confidentiality and safeguarding of clients.
8. Identify and attend appropriate training as agreed with the Chief Executive.
