**Virtual Ward Coordinator - Band 4**

**37.5 Hours per week, Permanent**

The team provide care to the acutely unwell who would otherwise be admitted to hospital, but for whom short-term treatment at home would be a better option. Treatment will include face-to-face assessment, care and monitoring, with daily consultant-led virtual MDT reviews for each patient
