An exciting opportunity has arisen for Assistant Practitioners to join a new service in the Virtual Ward team in South Nottinghamshire

Virtual wards are a national initiative and defined as “A Virtual ward is a safe and efficient alternative to NHS bedded care which is enabled by technology” NHS England and NHS Improvement.

Nottinghamshire Integrated care system, aim to deliver this through a Nottinghamshire wide provider collaborative, incorporating Sherwood Forest Hospitals, Nottingham University Hospitals, Doncaster & Bassetlaw Hospital, Nottinghamshire Healthcare and City Care to deliver an ambition of 400 - 500 beds across the Nottinghamshire by Dec 2023.

Virtual wards allow patients to receive care, remote monitoring and treatment they need in their own home, safely and conveniently rather than in hospital. Virtual Wards provide healthcare systems with a significant opportunity to delivery care through the collaboration of community services and acute hospitals providing an alternative to an acute admission and/or early discharge.

As a member of the Virtual ward Multidisciplinary team the role of Assistant Practitioner will support patient care to be delivered in the home providing an alternative approach to receiving hospital care or clinical monitoring. The Virtual ward positions whilst based at various sites deliver a joined-up approach across the Nottinghamshire Integrated Care System (ICS).

The overriding purpose of the Assistant Practitioner is to support the provision of the highest quality patient care to individuals whilst on the virtual ward pathway across a 7day period 365 days per year.

The post holder will have expertise with the patient group specialising in Frailty and acute admissions avoidance, and wider experience in which they practise which is underpinned by competency assessment and learning programmes.

The focus of the Integrated Teams is to maximise and enable independence of the individual and the role of the Assistant Practitioner for Rehabilitation is fundamental in supporting patients to achieve this. You will be expected to work without direct supervision undertaking tasks delegated by the relevant profession, in order to support the patient and the team and you will demonstrate excellent communication skills to maximise effective intervention to the patient.
