Temple House Practice is looking for a part-time Care co-ordinator to join our administration team.

Care co-ordinators play a key role in proactively identifying and working with people, including the frail / elderly and those with long term conditions, to provide co-ordination and navigation of care and support across health and care services. This includes making sure that individuals have access to the reviews and appointments they need to support them to manage their conditions effectively.

You will also support the Practice administratively with our safeguarding, and cancer care programmes. You will also support the Quality and Outcomes Framework (QOF), monitoring NHS contract requirements and local and national targets and undertaking workflow document management.

The successful applicant will be working 22 hours per week split over a Wednesday (9.15am-5.15pm, Thursday 9.15am-4.45pm) and Friday (9.15am-5.15pm).

Hourly rate is £11.53
