A Personal Adviser (PA) builds and sustains relationships with young people who are preparing to leave or have left care to ensure their views are taking into account and that practice is centered on meeting their needs.

**The role**

You will participate in assessment, preparation and review of care leavers’ pathway plans to equip them with the emotional, social and financial skills required for successful independent living. You will also ensure the leaving care service meets the requirements set through legislation and statutory guidance and any local or national protocols for all eligible, relevant and former relevant care leavers.

The Personal Adviser will co-ordinate the provision of services and take reasonable steps so that care leavers make use of these services and keep full, accurate and up-to-date records of contact with the care leaver and services provided using the Council’s electronic recording system.The kinds of issues where you will be the care leaver’s initial source of advice might include:

- To raise the standard of practice within Lewisham and improve the ‘Local Offer’ to care leavers.
- This role will provide greater capacity and relationship based working of all Lewisham care leavers aged 16-25 years; comply with the requirements set out in statutory guidance
- To prepare and support care leavers’ transition to adulthood to develop their independent skills.
- Provide a rapid and flexible response to supporting in crisis; which may include out of normal office hours and occasional weekend working.
- Maintain effective communication with partner agencies and other professionals in the development of services and to keep informed about care leavers’ progress and wellbeing.

You will have previously worked and engaged with young people and have demonstrable knowledge and understanding of adolescent behaviour and development. You will have excellent communication skills and possess sound organisational and time-management skills.

**Location**
Whilst the service is primarily based in Laurence House, 1 Catford Road, London, SE6 4RU. We operate a hybrid approach where you will be required to work both in the office and from home on a regular basis. You may also be required to work at or from any other buildings/locations.

£35,223 - £36,417 per annnum (SO1)
