**Role Information**:
You will be based within Oak Tree House, **Lancaster**, a supported housing scheme for people aged 18 or over with complex needs and at risk of homelessness, in Lancaster.

As a Recovery key worker you will have a crucial role supporting people to overcome the barriers they face.

You will work with people to establish their needs and help create a personalised action plan for change, providing advocacy, emotional and practical support and helping them move to permanent or specialised accommodation.

You will co-ordinate a package of support, liaising with relevant support services throughout.

You will complete regular reviews both in person and over the phone, ensuring that the customer’s action plan is delivered, helping improve their situation and empowering them to move forward with their life.
