Young Persons Support Worker

### Job Summary:
Care All Round - "Hope to Heal. The Spirit To Care". We are a small supported accommodation service offering step down support to young people aged 16-17. We work from an enablement approach supporting young people gain more autonomy to enhance their development as they approach adulthood. Young people are accommodated within their own studio flat within a building of 8 flats, offering floating support as and when required during the day as well as sleep in staff during the night (10pm-8am).
Young people's emotional resilience may be low at points with struggle to adapt, therefore, we seek motivated compassionate and dedicated Support Workers to join our team who are keen to listen and support young people build resilience.
As a Support Worker, you will provide essential support and assistance to young people enhancing their independent skills within supported accommodation prior to transitioning to adulthood (their 18th birthday). This role requires strong interpersonal skills, the ability to handle sensitive situations with empathy, and a commitment to improving the quality of emotional resilience and independent daily tasks for those whom you support.
## Duties:
- Provide personal support to young people in their flats and out in the community
- Assist with activities of daily living such as household maintenance, finances, employment, education or training
- Prompt medications according to prescribed schedules
- Prompt / support with nutritious meals
- Monitor and report changes in young persons ' physical and mental health
- Provide 1:1 therapeutic and/or development sessions with young people
- Assist / prompt household cleanliness
- Attend support young persons local authority care meetings
- Champion and advocate the young person’s wishes and feeling within reviews
- Building Maintenance and health & safety
- Follow and devise support plans
- Maintain accurate documentation of services provided to evidence outcomes
## Skills:
- Experience working with children and young people in a residential / support setting
- Strong communication skills with a focus on providing compassionate support
- Knowledge of first aid procedures and ability to respond to emergencies
- Proficient devising support plans and being innovative in achieving outcomes
- Familiarity with medication administration protocols
- Understanding of de-escalation techniques, managing behaviour
- Ability to work independently
- Ability to work with IT and efficiently grasp recording of data
