**Job overview**:
The Head of Transformation and Integration is an essential role within the Sutton Health and Care (SHC) senior leadership structure which enables health and care transformation and integration to happen in co-production with SHC staff, patients and with partner organisations and promotes sustainability across SHC, in association with the 2023-28 Group (Gesh) strategy, Sutton Provider Alliance and, also in association with the PLACE workstreams and expected delivery of the Integrated Neighbourhood Teams (INTs) agenda.

**Main duties of the job**:
This post holder will work in conjunction with the Sutton Provider Alliance principles and strategy in relation to plans to be taken forward yearly with respect of integrated transformation.

The post holder will have lead responsibility for SHC specific health and care developments, transformation and integration in the following areas:

- INTs - development and collaboration including the proactive care agenda
- Clinical service review for community nursing for SHC
- SHC Service Transformation Programmes (internal)
- Service Transformation Programmes which involve SHC from a SW London, Gesh, PLACE and Provider Alliance perspective (external)
- Other transformational/integration and collaborative programmes that become a priority for SHC as strategy and planning changes
- Development of bids completing tenders and awards as appropriate in conjunction with approvals from Gesh/Integrated Care Team meeting
- Cost Improvement Programme (CIP) to include income generation schemes.

**Detailed job description and main responsibilities**:
**Senior Leadership and Transformational Role**
- To lead on the INT collaboration programme for SHC as part of our key priorities of PLACE. This will involve working with each of the INTs - SHC and social care to collaborate and intertwin services around proactive care and the INT localism agenda
- To lead on clinical service reviews such as community nursing transformation and planned care and any other internal or external SHC programme work with transforms care and pathways
- To develop and lead on the programmes of work in year and each year for CIPs, transformation and integration as agreed by Gesh, PLACE, the Provider Alliance and SHC, holding both self to account and managers who are leading on key programmes of work
- To provide visible and supportive senior leadership and line management responsibility for all staff across areas of the portfolio, as well as supportive leadership for operational managers who are working with the post holder to take programmes of work forward
