Primary Care Mental Health Wellbeing Support Worker

An exciting opportunity working into various GP practices to support people with mental health needs within primary care. The post holder will work with adults who experience various mental health difficulties, delivering coping strategies and support. Support will be delivered from the primary care mental health practitioners working within the team and appropriate supervision will be delivered.
- To work alongside the primary care mental health practitioners to deliver plans of care
- To deliver low grade anxiety management / psychoeducation (Workbooks etc..] and Physical health promotion / the benefits of exercise
- To be involved in the assessment process
- To work alongside other agencies and be involved in MDT working
- Manage and prioritise a case load
- To be involved in service improvement/delivery

The primary care mental health team is a new service which consists of mental health practitioners who have worked in various different settings across mental health services. The staff team work into various GP practices across the locality and are an integral part of primary care working.

The Primary Care Mental health service offers Mental health assessment and short-term interventions to patients presenting with Mental health needs in Primary care. The service works closely with other organisations across the community 3rd sector, Access and Community services.
Access and record keeping in Primary care patient records systems ie EMIS

Wellness and Recovery training to include personalised care/ WRAP’s and coaching models.
The post holder will need to have attended training and/or understand Mental health needs and patient wellbeing ie. Learning disabilities, weight management, diabetes.
Have attended training or have equivalent knowledge of Graded exposure support and Behavioural activation and activity scheduling interventions.
Experience of working in a multi-agency environment and awareness of community services.
Experience of working with individuals with Mental health needs
Ability to contribute to service development and promote the role of the PCMHP service.

Experience of or willingness to attend training and contribute to support patients with;
- Low grade anxiety management / psychoeducation (Workbooks etc..] and Physical health promotion / the benefits of exercise
Behavioural activation and activity scheduling interventions, Graded exposure
Ability to work with and support patients with principles of self-management
Staff will attend a range of statutory and mandatory training applicable to all staff in the Trust and Primary care.
Over and above this the service offers specific in-house foundation/awareness level training on:
Ability to manage and prioritise a caseload
Joint working with other professionals and effective communication
Assessment of a person’s mental health presentation including Mental State Examination (MSE)
Understanding of psychological interventions and how to delver them
