**Job Purpose**
- To provide high quality personalised support service to older people, adults with physical disabilities, adults with learning disabilities, and adults with mental health problems including dementia, living in their own home
- To provide respite care for the carers of vulnerable adults
- To promote the independence, safety, inclusion, choices, dignity, privacy, respect and empowerment for all service usersof Cottage Care Services in their daily lives:
**Scope of the Post.**
- To provide support and implement all tasks in relation to individual service users as specified in the Service User’s person centred plan to a high standard in accordance with Cottage care Services policies and procedures
- To work in accordance with the SSSC Codes of Practice, the National Care Standards and Cottage Care Services Codes of Conduct.
- To protect service users from any form of abuse in accordance with Cottage Care Services Adult Support and Protection Policy

**Summary of Main Duties:

- **

**Personal Care, Medication Prompt, Meal Preparation, Assistance with life skills, Tuck down, Befriending and Escorting, Respite Care, Housework, Shopping, Bill Paying Recording of daily support accurately.**

**MAIN DUTIES AND RESPONSIBILITIES**
- **Personal care including, toileting, catheter care and Stoma Care, washing, dressing, oral care, skin care**

To support service users to maintain good standards of personal hygiene in accordance with the individual’s person centred plan ensuring that service user’s choices, dignity, respect and independence is promoted by enabling them to carry out personal care tasks as far as possible on their own.(See guidelines for dignity in personal care)
- To prepare meals for service users according to their needs and choice, observing Food Hygiene practice and policy at all times

- To promote good health, good nourishment, and well
- being by encouraging healthy eating as far as possible.
- To support service users in taking medication, promoting independence in this, but ensuring that all protocols are strictly adhered to and any risk is minimised as much as is practicable..
- To act as an advocate for service users by assisting in the development of person centred plans and by participating in meetings to ensure that the service user’s wishes are upheld.
- To contribute to accurate and detailed record keeping whilst maintaining the service user’s privacy in accordance Cottage Care Services policy of confidentiality, dignity and respect.
- To, under guidance of the management and where appropriate
- liaise with the service users relatives, social work care managers, G.P and other health professionals,
- Undertake appropriate staff development and training programmes to improve personal skills and knowledge
- To attend weekly staff training meetings and regular supervision with the manager.
- To observe safe systems of work, implement moving and assisting training when carrying our personal care/transfers and report any risks to management.
- To promote independence by enabling service users to complete the tasks of daily living as independently as possible ensuring they are as free from harm to themselves or others as is practicably possible.
- To adhere to all risk assessments and highlight to management when a re-assessment of risk is needed.
- To ensure that the service users are supported in maintaining all aspects of their tenancy including payment of utility bills
- at all times adhering to the Cottage Care Financial Procedures Policy.
- To assist service users with managing their finances -strictly adhering to all Cottage Care Services policies
- and promoting independence in this as much as practicable.
- To support service users to attend appointments, to escort for leisure, and to assist our service users to engage in meaningful activities, and be involved in community life.
- To challenge appropriately any discriminatory practice aimed at our service users, and report this to management.
- To support service users in maintaining a home environment that is clean and tidy and is free from health and safety related hazards.
- To liaise with managers, staff and multidisciplinary teams to ensure that changes to the environment are made where necessary to promote an accessible and functional home for the service user -whilst always ensuring that this environment reflects the service user’s needs and wishes.
- To carry out all household checks in accordance with Cottage Care Services policies.
- To carry out any other tasks as requested by the management.

**Job Type**: Part-time

**Salary**: £10.90-£11.15 per hour
