We have an exciting new opportunity for **Complex Needs Coordinators** to join our team at **Dinardos Care & Support Ltd**. Dinardos is a social care provider specialising in supporting Adults with challenging and complex needs.

As a Complex Needs Coordinator, you will provide a critical role in supporting the existing team in the day-to-day support of those residents that have been identified as having multiple and complex needs, based on the following definition:

- Complex Needs (sometimes referred to as ‘multiple needs’ ‘chronic exclusion’ or ‘severe and multiple disadvantages’) was defined as when a person experiences a combination of the following domains:_
- Homelessness (or high risk of becoming homeless)
- Substance Misuse
- Mental Health
- Offending Behaviours
- Learning Disability
- Physical Disability
- Childhood Trauma
- Child Sexual Exploitation
- Domestic Abuse

**The Role**:
To work as part of a regional complex needs team that coordinates and delivers day-to-day support to residents that have been identified as having multiple, coexisting, and complex support needs. This includes ensuring that residents are appropriately andfairly assessed, leading on resident involvement initiatives. You will also contribute to the formulation and delivery of a programme of resident activities.

An additional key element of your role would be to enhance the skills of other members of staff and support them in the context of group as well as individual mentoring.

**About you**:
You will have experience in the homeless sector (or a similar setting) as well as dealing with challenging behaviours. You will require good liaison skills with other agencies e.g. health and social care, substance misuse and the Department of Work and Pensions.You will be working primarily with people who have traditionally faced barriers to accessing support, including those from a wide variety of underrepresented groups such as BME communities; people who have problematic substance misuse issues; residents withlanguage and immigration barriers; and socially excluded groups such as those who are street homeless.

You would also need to exhibit some knowledge of relevant legislation, for example, Housing legislation, Rough Sleeping Strategies & the Mental Health Act. and new approaches to, or models of support, for example, Trauma-informed approaches, recovery modelsand stages of change. To be successful, you will have a good understanding of risk management and safeguarding in relation to people who experience rough sleeping as well as being naturally motivated, adaptable and a committed member of the team.
