An exciting opportunity to join the Crisis Assessment and Response team as a Patient Support Worker has arisen. The post holder will provide care to people who are in crisis to return to their usual level of activity (baseline) before requiring support. Where this is not possible the team will identify further support and/or long-term care needs and make appropriate referrals.

The Crisis Assessment and Response team (CARe) is looking for Patient Support Workers to join our community based team.

The CARe team supports individuals in their own home, or usual place of residence with the overall goal of preventing hospital admissions and supports individuals who are in crisis to return to their usual level of activity (baseline) before requiring support.

The Crisis Assessment and Response team works alongside Oxford Health as part of the Urgent Community Response pathway and responds to urgent two hour referrals.

The team operates 7 days a week from 7am-10pm and our staff work a mixture of early (7am-3pm) and late (2pm-10pm) shifts, including every other weekend. All staff have a set 4 week rota. Full time (37.5hr) and part time contracts are available

The team covers the whole of Oxfordshire, so it is essential that the post holder has a full UK driving licence and a reliable car available for work.

The CARe team has the following two bases, Unipart Business House and Horton General Hospital. You will be assigned to the base closest to where you live, for mileage purposes but will be required to cover the whole of Oxfordshire.

If you are shortlisted for this post, you will be required to complete a pre-interview assessment and will only be invited to interview, if you score over 70%.

The Trust comprises four hospitals - the John Radcliffe Hospital, Churchill Hospital and Nuffield Orthopaedic Centre in Headington and the Horton General Hospital in Banbury.
