Totally was established to support the NHS and ensure that patients could quickly access the healthcare they need. We partner with our healthcare commissioners across the UK & Ireland to deliver services that help them manage demand and reduce waiting lists. We deliver services for, Urgent Treatment Centres, NHS 111, GP out of Hours and a National Clinical Assessment Service from our Urgent Care Division.
**Your day to day**
Work as Care Navigator/Patient Champion to effectively manage patient flow in a fast-paced Urgent Treatment Centre (UTC) Service. Working as part of a team consisting of doctors, nurse practitioners, paramedic practitioners and receptionists in a demanding environment providing a professional and courteous response to patient requirements, ensuring safe and seamless patient transfers between the different areas of the urgent and emergency care service with adherence to Totally values and procedures.
The role will involve Encouraging members of the public to access the appropriate healthcare/services through providing information about services available and how to access them. The Care Navigator/Patient Champion will inform and enable people from different communities to find the right source of help for themselves and their families to deal with minor ailments.
The role will be based at the Scarborough UTC, will be patient and carer focused and will help UTC staff to communicate the appropriate use of Urgent Care.
You will:
- Ensure all staff report for duty, follow the organisational procedure in case of staff absence or lateness.
- Ensure Clinical Shift Leads are highlighted on the board and work hand-in-glove with Shift Lead to optimise clinical roles allocation
- Working closely with the Streaming Nurse, identifying urgent patients to be taken to UTC
- Ensure all staff have appropriate systems log ins, post generic log ins, and troubleshoot as needed.
- Monitor the UTC waiting room consistently and alert the Shift Lead or clinical member of staff if a patient is visibly deteriorating or requires urgent clinical review
- Monitor patient lists
- Educate patients on the appropriate use of the UTC as per pathway and UTC leaflet
**Qualifications, Experience & Training**
- Good general education, educated to NVQ 2 level or equivalent
- Working knowledge of Microsoft Office including Word, Excel and Outlook
- Knowledge of medical terminology
- Prior work in a fast-paced environment
- Direct patient/customer facing experience
- Working with Multi-disciplinary teams using own initiative to deliver aims and objectives within the role
- Demonstrable experience of working in an reception/administrative environment and working with computerised data systems
- Experience of using filing systems
