Primary Duties and Areas of Responsibility Client Engagement and Goal Setting: Take referrals from and make referrals to a wide range of agencies within primary care networks, building proactive team links within the wider PCN team. Meet people on a one-to-one basis, within an identified work environment or public area where appropriate within organisations policies and procedures. Giving people time to talk and focus on What matters to me. Taking a holistic approach based on the persons priorities and the wider determinants of health developing trusting relationships.

Using motivational interviewing techniques to develop personalised action plans and goal setting, empowering them to take control of their wellbeing and improve their health outcomes. Enabling and empowering people to achieve their goals by supporting them to develop and work towards Specific, Measurable, Achievable, Realistic, and Time appropriate steps and recognise achievements. Service Access and Feedback: Develop a strong awareness and understanding of when it is appropriate or necessary to refer people onto other health professionals/agencies/teams, when what the person needs is beyond the scope of the role. Link people to local community groups and services, empowering them to participate in community groups.

Be proactive in developing strong links with all local agencies to raise awareness of social prescribing and how partnership working can reduce pressure on statutory services, improve health outcomes and enable a holistic approach to care. Outcome Measures and Data Capture Provide comprehensive monthly data capture using standardised outcome measures Wellbeing Star, Star Online, Short Warwick-Edinburgh Mental Wellbeing Scale (SWEMWBS) Provide 1 anonymised case study per month demonstrating referral concerns, goals setting and outcome actions to support service evaluation. Record Keeping: Maintain timely and accurate records of client interactions, outcomes, and referrals, within the expected timeframe for efficient data management.
