Wirral All Age Continuing Care service is looking for a skilled, motivated & experienced registered nurse to join a dynamic team of nurses that provide commissioning services for people with complex needs eligible for NHS Continuing Healthcare funding or NHS-funded nursing care.

A key aspect of this post involves line management and supervision of Individual Commissioning Nurses and supporting other members of the service, staff induction and development and working closely with the Clinical Lead and local partners including health, social care and independent providers.

Experience of undertaking assessments, and working with patients, families & carers is essential. As Clinical Team Leader in the CHC Team the post holder will support the team in driving key areas of service delivery through involvement in specific pieces of work which may include complex and sensitive information and supporting CHC Quality, Innovation, Productivity and Prevention (QIPP) Improvement plans.

Main duties are line management of clinical staff including the nursing team. Ensuring implementation of the National Framework. Working closely with the Clinical & Operational Leads to support the management and assessment processes and patient flow. Working closely with the whole service and local partners and other key stakeholders to ensure people across the Wirral are screened and assessed appropriately. Supporting processes such as complaints, appeals, disputes and working with the hospital & hospice teams to support fast track pathways.
A valid driving licence and access to a vehicle for business purposes is essential. Whilst the role is based on the Wirral, there may be times when it is necessary to travel further afield as part of the role to see patients placed in other areas of the northwest / country.

New entrants to the NHS will commence on the minimum of the scale stated above.
