This role is new to the Trust and the postholder will be responsible for assisting with the safe transition of patients who are fit to be discharged from hospital back to their homes.

Please note, the Trust is unable to offer visa sponsorship for this role.

Providing high standards of care to adults in their own homes requiring supportive care to live safe, healthy and fulfilled lives. This will include providing support with personal care, meal preparation and administration of medication.

- Assist the client in activities of daily living, promoting client comfort
- Deliver plans of care & safeguarding vulnerable people
- Contribute to outcome focused reviews
- Encourage client’s independence, choice, health & wellbeing
- Encourage client’s use of equipment & assistive technology
- Facilitate clients’ transition to alternative services & support if needed
- Engage in organisational learning and development and continuing professional development
- Provide excellent client care and support
- Recording Care - complete documentation and feedback

The Domiciliary Care Team will provide Domiciliary care to patients at their own home in the community, upon hospital discharge. Some clients will be suitable for re-ablement support and this role will be aligned to this patient group.

- To work unsupervised in client’s homes in the community
- To provide personal care and support to clients, e.g. assistance with personal hygiene (including bathing and dressing needs), eating and drinking, mobility needs under direct or indirect supervision
- Assist, support or prompt the client in managing medication
- Support and assist clients in managing long term medical conditions and to provide the appropriate care and support to meet the individual client’s plan of care
- To assist with domestic tasks and support daily living skills
- To build client confidence to attend social activities
- Nutrition assessment and provision of meals; including cooking and dietary advice
- Encourage and inform clients in managing their own personal safety, and health and wellbeing
- Support, guide and enable clients in decision making scenarios and where appropriate refer to management or other professionals
- To report accurately perceived changes in a client manner, observation, eating habits etc.
- To monitor and report any changes that identifies the need for review of client’s care
- To escalate in accordance with policy any safeguarding concerns and promote the welfare of vulnerable adults
- To promote health within the community setting
- Referral to specialist health and social care teams
- Carrying out routine visits
- Ensure comfort, safety and dignity whilst also maintaining confidentiality at all times
- There will be a requirement to provide weekend and out of hours service delivery
- May be required to support clients with shopping, including monitoring the security of the client’s home i.e. key holding and security of valuables
- Make judgments involving straightforward job-related facts or situations
- Assisting qualified staff
- Able to recognise signs of a deteriorating client and alerting qualified staff accordingly
