The Wellbeing Hubs are a network of 6 community spaces run by a local partnership to provide a welcome, safe and confidential space for people to access a wide range of support for issues and concerns that are affecting their wellbeing. The Hubs are a one stop shop where people can confidentially work through any issues they are facing and be supported to get the right services and support quickly, including managing stress, mental or physical health, substance misuse, financial worries, domestic abuse and relationship issues.

The Bridge Project is delivering one of the main Hubs, which is located at the Yorkshire Cancer Support Annex Building and is the base for several VCS and statutory staff who provide non-clinical support to hospital. This is an exciting project aimed at delivering an integrated team model and to allow ‘streaming’ directly from the Emergency Department into a non-clinical therapeutic space.

**We are looking to appoint a full-time Wellbeing Hub Connector and Triage Worker. **This is a fixed term contract until 31 May 2024, with the possibility of extension depending on funding. The postholder will be based at the City Wellbeing Hub, however it is essential that the successful applicant is able to travel to a sister Hub site 1 day per week and when required to provide cover at another site in Bingley.

**You will provide the first point of contact for people referred to the Wellbeing Hub and identify the most effective pathway for support. **This involves adopting a person-centred approach to unpick a client’s wellbeing issues and connect them to the relevant services. The goal of the triage is to help clients access support that will enable them to make improvements to their general health, wellbeing and social situation.

We are looking for someone who is passionate about empowering people to achieve positive health and wellbeing, who has experience of working within the health, care or voluntary sector. You will have a good knowledge of the range of health issues affecting health inequalities and be able to communicate with a wide range of people in a sensitive and non-judgemental way. You will provide a high-quality service by having a positive approach whilst providing advice, information, and support along with facilitating wellbeing groups.

The successful applicant will have an intermediate working knowledge of Microsoft Office packages in-order-to maintain electronic case records, statistical monitoring and updating the existing database, whilst maintaining confidentiality and GDPR compliance. You will be highly organised, be able to work independently and manage your time effectively.

There is an element of flexibility around working hours required by the postholder. A willingness to undergo an enhanced DBS check is also a requirement of the post.
