To work without direct supervision but under the direction of the AHP (Physio, OT, SALT, Dietetic) to perform specific tasks for patients as part of their treatment and rehabilitation programmes. To provide initial assessments and follow-up treatments to patients, identify patient needs, develop treatment plans and review progress regularly. Ensure patient centred goals are agreed and documented in their goal booklet. Communicate with the MDT any patient issues, progress and changes in the patient condition.

To organise, prioritise and adjust own workload in relation to demand and time constraints. The ability to make changes to own practice, offer suggestions for improving services and engage with research and audit Be responsible for maintaining stock levels including feed and nutritional supplements. Be responsible for ordering, maintaining and cleaning of equipment You will be expected to work your contracted hours over 7 days. Provide Post ICU support: follow up reviews, monitoring, highlight any on-going rehab, nutrition, swallow or speech issues.

Patient Care To keep the patient at the centre of his or her care, ensuring patient and carer participation in decision making. Develop person specific goals. To demonstrate specialist rehabilitation/ therapeutic moving and handling skills for ITU and HDU patients. Assist in cognitive assessments of patients.

Consider patients mental well-being and take opportunities to orientate patients. To communicate effectively with patients and carers. To be able to motivate and persuade patients despite barriers to communication e.g. delirium, confusion, sedation, ventilation, pain, hearing loss.

**To complete specific assessments with patients such as**: Activities of daily living (including washing and dressing), weight ,height and grip strength, Muscle strength testing, sensory assessment, local nutritional screening tool, CPAX and other outcome measures. Ensuring adequate Oral intake support: e.g. helping with meals & supplements, ensuring food charts are in situ and completed. Carry out rehabilitation programmes for patients with communication difficulties, including making and demonstrating low and high tech communication aids.

Share with other staff ideas for communication aids. To carry out recommendations and therapy with patients with eating, drinking and swallowing difficulties To measure, issue and fit appropriate aids/equipment and instruct patients on their safe use eg specialist seating, cushions, splints. To be able to use technology to enhance patient treatment and experience e.g. Ipads, photos of patient experience, virtual access visits, Communication aids such as eye gaze.

To be actively involved in board rounds, MDTs and case conferences. To maintain a high standard of accurate, comprehensive documentation in the multidisciplinary notes in line with legal and team requirements. Complete on-going patient referrals to other services following discharge from ITU. Collect other relevant clinical information eg biochem, feed delivery, blood glucose, bowels, insulin, Propofol and fluid balance Frequent exposure to highly emotional circumstances including critically or terminally ill and those with chronic long-term disabilities.

There is daily contact with bodily fluids and a little contact with fleas and lice. There may be exposure to physical aggression. You may need to treat patients in isolation, requiring you to wear full PPE often for prolonged periods of time. Responsibilities for People or Training To be involved in the induction, training, development and supervision of other staff and students.

To train patients and carers in aspects of the patients treatment to ensure optimal patient outcomes.
