As part of this growing team of Care Connectors who work with social workers, nurses and providers to assist people to secure a variety of care and support options to meet their needs. This team works closely with our Adult Social Care teams to enable people to live a great life in Cheshire West.

You will have the opportunity to;
- Be part of an expanding team that is being invested in to ensure we are providing the best care and support options for local people
- Work closely with our Adult Social Care and Hospital teams to design person centred support options with the people we support.
- Develop close working relationships with our great care providers across the Borough, assisting them in coordinating care in the most efficient and effective way
- Work with a whole variety of services including small/medium businesses (micro enterprises), technology providers and our vibrant community and voluntary sector, helping to curate the options available for local people.
- Be central to a fast paced and dynamic environment where your role will be pivotal to the success of key health and social care outcomes.
- Be a lynchpin, bringing together contracting, commissioning, operational and financial services across the Council to deliver a seamless and joined up Care Connections service.

You will have:

- experience of working within social care, health and/or contract management disciplines
- the ability to successfully negotiate within a competitive market environment
- a proven track record of working in a fast paced and pressurised environment delivering speedy outcomes
- Excellent customer service skills
