This is an exciting time to join the urgent Crisis Response Team (CRT) with admission avoidance being a local and national priority following the launch of the NHS Community health services 2-hour crisis response guidance. We are a proactive, dynamic multi-disciplinary community team, providing care to Stockport residents in their own home, using new technology and remote devices to support patients in their own home.

As a SSW you will be follow nursing and therapy care plans within agreed parameters, delegated by the registered practitioners to ensure patients remain as independent as possible and achieve the best possible health, therapy and social outcomes.

This is a rewarding role which will give you the opportunity to utilise your senior support worker skills. Training opportunities are available to develop physiological observations, escalation competencies and therapy interventions.

Education and ongoing support are important to us and registered practitioners facilitate bespoke training and education days for the SSWs in the team, along with providing robust ongoing mentorship and support.

The post holder will be expected to work within the multi-disciplinary Crisis Response Team that deliver community based services to a GP Practice population, undertaking higher level nursing, therapy and social care tasks within agreed parameters, delegated by the registered practitioners. The job entails home visiting and lone working without direct supervision. The scope of the role will focus on supporting admission avoidance, early supported discharge and promotion of independence whilst assisting in the effective management of patients with long term conditions.
- To undertake duties, both therapy and nursing in the patient's own home, following the gaining of competencies and where deemed appropriate by a registered professional.
- Provides emotional and physical support to patients and carers with chronic illness and terminally ill
- To monitor the well-being of people within current case load, contributing to protecting those individuals whose health and well-being maybe at risk
- To deliver outcome and reablement focused support and assist in the support and education of family/carers whilst maintaining confidentiality

If successful this vacancy will require an enhanced DBS check at a cost of £41.90. This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £13.
