**Job summary**

Full time (or part time mix) vacancy working for the Weymouth and Portland Primary Care Network (PCN) providing care for patients across the locality in a community and practice setting, practice office based. Working under the supervision of the named GP of the patients with additional support from a dedicated GP Mentor/Supervisor.

**Job description**

**Job responsibilities**
- Attending patients as first clinician or follow-up, in practices and in patients own home
- Take medical histories from patients
- Carry out physical examinations
- See patients with undifferentiated diagnoses
- Perform diagnostic and therapeutic procedures
- Request and interpret diagnostic studies
- Develop and deliver appropriate treatment and management plans
- See patients with long-term chronic conditions
- Formulate differential diagnoses and management plans
- Provide health promotion and disease prevention advice for patients
- Opportunity to develop and shape this service
- Opportunity for further training, e.g. prescribing when it becomes available
- Perform any duty specifically delegated by the Federation Management as being properly the responsibility of this post.

**Person Specification**

**Experience**

**Essential**
- 12 month post-qualification experience
- Working in a healthcare setting as part of a team
- Accurate recording of data

**Desirable**
- Primary or secondary care experience

**Knowledge**

**Essential**
- Good working knowledge of primary care and the challenges facing patient care in the NHS.
- Understanding of medical terminology and knowledge of anatomy & physiology

**Desirable**
- IT GP clinical system, SystmOne

**Skills**

**Essential**
- Understanding and adherence to strict confidentiality
- Excellent communication and interpersonal skills, both written and verbal
- Good teamworking skills
- Able to analyse & extract relevant information

- Physician Associate Postgraduate Diploma/Masters from an accredited UK University.
- Passed the Physician Associate National Examination.
- Member of the Physician Associate Managed Voluntary
- Register (PAMVR)
