**Overview**

The job holder will support service users to progress towards individual goals, help them to work together in centre and develop an individual sense of social interaction.

**Main duties**
- Ensure the service users voice is included at all stages.
- The job holder will be required to be a key worker for nominated service users, ensuring their relationship within and external to Teamwork are understood and nurtured to provide the best possible service to the individual
- Identify individual opportunities to support service user progression, providing all care and support to enable individuals to maintain their independence and to achieve their goals.
- Ensure that policy and procedure relating to safeguarding, serious concerns and formal complaints are followed in the immediate instance. Document and notify relevant parties and inform a senior manager as appropriate.
- Carry out and implement relevant risk assessments where needed with support of your manager.
- Ensure records are immediately updated to reflect changing need. Provide support to your line manager to evidence that changes are effectively communicated.
- Supervise the effective, timely and accurate reviewing of individual and service records.
- Monitor progress of service users whilst building positive relationships with relatives and advocates.

**Salary**: £20,500.00-£22,000.00 per year
