**Job overview**:
We are looking for a enthusiastic motivated individual's to join our expanding Prestwich District Nursing Team based in a community setting. This is an exciting opportunity to be involved in the integrated health and social care organisation.
You should have excellent communication and interpersonal skills which will be utilized with in this multi-disciplinary health care team; You must be multi-skilled and be able to work across various disciplines.
You must be proficient in providing planned care and be able to take appropriate action in achieving set goals with minimum supervision.
**Main duties of the job**:
You will carry out screening/therapy/nursing procedures as identified within their role.
The Assistant Practitioner will provide supervision and assessment for Healthcare Support Workers and below undergoing relevant training.
You will be given a dedicated induction programme and a full training and development plan will be offered.
You will have an NVQ level 3, a formal Assistant Practitioner qualification and be practising as an Assistant Practitioner. We welcome forward thinking individuals who would like to be part of a dynamic rapid response team covering Bury.
Due to the nature of the post it is expected that you will have a full driving licence and will have regular use of an appropriate vehicle (as determined by the Trust).
