**MAIN PURPOSE/ROLE SUMMARY**:
The post holder will be an experienced paramedic, who acting within their professional boundaries; will work closely with GP practices and the multidisciplinary team (MDT) within the Torridge Health Primary Care Network (PCN). The role is pivotal in ensuring all patients receive the best possible care and service. The post holder’s role will support the clinical director and member practices in coordinating and delivering all key activity including access to services, advice and information, ensuring a patient-centred approach with timely and effective care planning and delivering effective clinical care. The role will include supporting digital initiatives and coordinating the patient journey through primary care with focused work around the delivery of the Directed Enhanced Service (DES) specifications to include Enhanced Health in Care Homes (EHCH).

**CORE RESPONSIBLITIES OF THE ROLE**

Key responsibilities for the post holder in delivering the additional PCN health services to patients will include but are not limited to the following:

- **1. MANAGEMENT OF PATIENT HEALTH AND ILLNESS**

1.1 To work with GPs and members of the MDT within the PCN to identify and clinically manage a case-load of patients.

1.2 Provide a point of contact within the PCN for patients residing at home or within Care Homes who present with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision-making, to establish a diagnosis and management plan in the patient’s home.

1.3 To work closely with and in partnership with community providers, care home staff and other partner organisations to help improve patient outcomes ensure better access to healthcare and help manage general practice workload. The role has the potential to significantly improve the quality of care and safety for patients.

1.4 To evaluate clinical information from examination and history taking. Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports, liaising and sharing information with the MDT.

1.5 To support the Clinical Director and member practices in the delivery of the DES specifications.

1.6 To prioritise health problems and intervene appropriately, including initiation of effective emergency care.

1.7 To provide coordination and navigation with the aid of digital tools for patients and their carers across health and care services.

1.8 To provide safe, evidence-based, cost effective, individualised patient care.

1.9 To support the coordination and delivery of MDTs within the PCN.

1.10 To maintain accurate and contemporaneous records, utilising computer systems where appropriate and consider the Caldecott Principles and GDPR Regulations in relation to all data handling.

2.4 To work within the Paramedic Code of Professional Conduct.

2.5 Participate in organisational decision making, interpret variations in outcomes and use data from information systems to improve practice.

2.6 To maintain own professional development in line with professional Regulations.

2.7 To promote evidence-based practice through the use of the latest research-based guidelines and the development of practice-based research.

2.8 Monitor the effectiveness of their clinical practice through the quality assurance strategies as the use of audit and peer review.

2.9 Participate in continuing professional development opportunities to ensure that up-to-date evidence-based knowledge and competence in all aspects of the role is maintained.

2.10 Keep up to date with relevant health-related policy and work with the MDT to consider the impact and strategies for implementation.

2.11 To play an active role in the development of PCN development plans and new services.

2.12 Demonstrate team leadership, resilience and determination, managing situations that are unfamiliar, complex or unpredictable and seeking to build confidence in others.

**3. TEAM ROLE**

3.1 To liaise with all members of the PCN Team and other agencies - local authority, social services, secondary care and voluntary sector in order to assure appropriate care is provided for the Care Home patient population.
