Health and Homelessness Link Worker

We require a Health & Homelessness link worker who can transform our Bury Red Door day centre into a Health promotion Hub with outreach to a service in another Greater Manchester borough through:

- Carrying out health promotion activities
- Training and supporting Volunteer Health Champions to support the project
- Setting up new health partnerships and expanding/consolidating existing ones
- Promote and support on site health drop ins and increase uptake/attendance
- Encourage and support people to register with mainstream health services such as GPs, dentists and to help encourage people to attend appointments or be accompanied if needed
- Monitor and evaluate improved uptake of health services and outcomes

This is an exciting opportunity to be involved with this new project, which will help organise health promotions in the day centre for people who are homeless or at risk of homelessness, as well as signposting and supporting people to access health services.

The role requires you to communicate effectively on a one-to-one basis with people and in groups in a wide range of contexts, as well as working independently to use your own initiative to decide on appropriate measures for the health hub. Good IT and administration skills is essential as you will be monitoring and evaluating all aspects of the health hub by collecting qualitative feedback and updating accurate records on our databases.

We are looking for someone who has knowledge and experience of working with people who are homeless or at risk of homelessness, or who have drug/alcohol or mental health issues, as well as promoting health care/wellbeing to excluded/hard to reach groups.
