**Compass Medical Practice**
Compass Medical Practice is a virtual GP Practice for patients who have been removed from mainstream GP practices as they are deemed ‘difficult to manage’. The practice covers a wide geographical area which incorporates Lancashire, Cumbria and Halton. Patient consultations take place by telephone /video link and if a face-to-face consultation is required, the appointment is undertaken within a clinical space within the patients’ community or the bespoke Compass Medical Practice clinical van. A risk assessment is undertaken prior to all face-to-face appointments and there is a minimum of one patient liaison officer present for each appointment.

**Job Summary**
Social prescribing helps patients to improve their heath, wellbeing and social welfare by connecting them to existing community services, resources and facilities, wherever possible and applicable. The Social Prescriber will have insight into the wider determinants of health and will work with individuals to create a shared plan, based on what matters to them to improve health and wellbeing. You will help them to connect with community and voluntary groups locally, supporting them to make their own choices and help them build confidence to cope with social situations, such as community group meetings. This could be a whole range of activities, including bereavement groups, dementia cafes, art classes, debt management services, gardening clubs, physical activity groups, fitness classes and many others.
**Duties and Responsibilities**
- Undertake assessments with patients to gain an understanding of their life, health condition and how it affects them, and to explore options for social support.
- Connect patients to local support and services and supporting them to make contact.
- The social prescriber will also encourage patients to be compliant with clinical interventions, medication and participate with National Screening Programmes.
- Patient outcome measures will be assessed via the ONS Wellbeing Scale, the Patient Activation Measure (PAM) and case studies.
- The social prescriber will support patients who are assessed as ready to return to a mainstream GP Practice and who need input through the step-down process.
- Undertake follow up conversations and where applicable, supported visits to ensure that patients have been able to access appropriate services.
- Attend a range of multi-agency/ multi-disciplinary meetings.
- Be proactive in seeking out new community support and activities for patients to access.
- Participate in meetings/events to promote social prescribing and population health.
- Report any Safeguarding concerns to the appropriate person.
- Ensure that Social prescribing codes are input into EMIS, adhering to data protection and data sharing guidance.
- Ensure that all activities are monitored and evaluated.
- Attend internal meetings as required.
- Work in accordance with FCMS policies, including the equal opportunities policy and practice.
- Attend any training courses and supervision sessions as required.
- Undertake any other duties as may reasonably be required from time to time.

- Work to attain the service KPIs and targets
- Ensure all patient contact is recorded in a timely manner on the IT systems to enable tracking of patient outcomes, both in real-time to support ongoing intervention planning and retrospectively to understand the impact of the service
