Health and Reablement Assistant

There are 4 posts available with hours between 22.5 and 37.5 available. The posts will involve rotational shift system to cover shifts across the 7 day / 24 hour roster. ‘Flexible’ working arrangements will be considered. Any request will be set against genuine operational requirements and limitations and it is not obligatory for a manager to accede to the request.

Health and Reablement Assistants will become a valuable member of the inpatient multi-disciplinary team and will support and oversee the provision of high quality patient focused care under the guidance of the qualified nursing and therapy teams.

The post holder will enable personal care using an individualised approach, promoting independence by following the rehabilitation / reablement goals programme established by therapy colleagues. As part of the ward team the post-holder will create opportunities for recovery and rehabilitation for patients, supporting achievement of their goals and return to their community, more specifically the post-holders will:
Provide leadership and support for care staff and act as a champion for service users’ safety and dignity.

To work alongside the inpatient nursing and therapy team to assist in the delivery of care as identified in the nursing care plan and the rehabilitation / reablement program provided by the therapist.

To promote the ethos of enabling patients to be as independent as possible and in line with their discharge destination plan and goals.
