Hours: Fixed term 1 year contract, Full Time (37.5 hours)

**Salary**: £23,400 p.a

Location: North & Mid Hants Primary Care Networks (which includes, but not limited to; Andover, Winchester and Basingstoke)

This is an exciting opportunity to work in a brand new, fixed term role across GP surgeries in North & Mid Hampshire. You will be employed by Andover Mind to work closely with the Primary Care Networks in Hampshire to support those with Serious Mental Illness (SMI) to take up their annual health checks and provide holistic wrap-around support.

As an SMI Health Facilitator, you will be liaising with GP surgeries to identify those on the SMI register and will contact these patients to encourage and facilitate in booking their annual health checks. You will be expected to provide the health checks to patients, particularly in an outreach capacity, to those who may not have engaged with their health checks previously. You will be required to record your notes and data onto GP systems. You will work with patients who need more ongoing mental health care to provide a holistic care plan including onwards referrals and signposting.

Applicants will need a full driving licence and access to a vehicle for business purposes.
