A vacancy has arisen in Tregaron Community Hospital for a motivated, enthusiastic, forward thinking and patient-centred individual to join our ward team. Tregaron Community Hospital has a 15 bedded general medical ward, specialising in care of the frail and elderly and palliative care. We are a motivated team committed to the delivery of a high standard of care we can be proud of for patients from across Ceredigion.

We are looking for a Family Liaison Officer who is able to work well as part of a multi-disciplinary team, is caring and kind and who wishes to improve patient experience. You will be expected to work as delegated to by the Site Administrator and Ward Sisters, supporting patients to stay in contact with their family and friends and actively engaging with them when they have no visitors.

The role is an evolving one and you will be trained, supported and supervised to work as part of the team to promote patient safety and comfort more widely and support the smooth organisation of the ward. We have a permanent role to offer on a part time basis.

The successful applicant will be kind, caring, polite, conscientious and hard working as well as willing and able to learn. They will be a team player with sound communication skills who can contribute actively to service development.

You will organise patient visiting and liaise directly with the ward team, patients and their families and friends to promote a welcoming environment which best ensures patient safety.

You will spend time with patients, alone or in groups, to offer meaningful activity such as talking with them, setting up multi-media for their entertainment, quizzes etc.

You will help the ward team with simple direct care tasks that you will be trained and supported to undertake for example helping patients choose their meals and giving out meals.

You will support the smooth running of the ward with simple administrative tasks that you will be trained and supported to do such as answering the phone and helping with stock management.

You will support the team to know how they are doing by engaging with patients in experience and satisfaction questionnaires and helping with audits as trained and supervised to do; and displaying the results.

You will be a member of the core team and contribute to handover meetings as well as staff meetings which promote a safe and caring environment.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Hywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire and its bordering counties. Our 11,000 members of staff provide primary, community, in-hospital, mental health and learning disabilities services for around 384,000 people across a quarter of the landmass of Wales. We do this in partnership with our three local authorities and public, private and third sector colleagues, including our volunteers, through:
**Four main hospitals**: Bronglais General in Aberystwyth, Glangwili General in Carmarthen, Prince Philip in Llanelli and Withybush General in Haverfordwest;
**Seven community hospitals**: Amman Valley and Llandovery in Carmarthenshire; Tregaron, Aberaeron and Cardigan in Ceredigion; and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire;
48 general practices (four of which are managed practices), 47 dental practices (including three orthodontic), 99 community pharmacies, 44 general ophthalmic practices (43 providing Eye Health Examination Wales and 34 low vision services) and 17 domiciliary only providers and 11 health centres;
Numerous locations providing mental health and learning disabilities services;
Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.

Your immediate line manager will be the Site Administrator. The Ward Sisters will also be responsible for your support, training and supervision.

You will be trained and supported to adhere to core infection prevention and control policies, data protection policies and others which affect every aspect of patient care.

The ability to communicate in Welsh is desirable for this post. If you do not meet the Welsh Language requirements specified, the Health Board offers a variety of learning options and staff support to help you meet these mínimal desirable requirements during the course of your employment with us.
