_Rehab Direct was established in 2012. Originally to deliver rehabilitation and support in a different and more innovative way for injured claimants following personal injury. We have specialised in the management of patients where chronic pain is the fundamental problem._
- More recently we have been delivering primary care pain services into the NHS and have developed a bespoke, modular approach to support individuals presenting with persistent primary pain._
- We are looking to recruit to the post of Health and Wellbeing Coach, to work within our Pain Management Team, providing personalised support to people referred by team members and local agencies. Health and Wellbeing Coaches are critical to the delivery of this service._
- Health and wellbeing coaches help clients to assess their lifestyles and wellbeing, set goals for improving their health, agree action-plans, and provide practical support_
- This is a vital role within the Primary Care Network and the post holder will provide health and wellbeing coaching for vulnerable people in the community helping them to manage their persistent pain, often with other long-term conditions. They will work closely with pain team colleagues, primary care teams to manage their own caseload_
- The role will support individuals to feel in control of their health and wellbeing by addressing mental health, general wellbeing and inclusion needs using a behaviour change approach and active recovery methodology._
- This post would suit individuals who have worked with patients in roles such as therapy assistant, rehabilitation assistant and personal trainer._
- The role is based in our Sheffield office and the remote service is delivered by a range of modern media platforms._

**Job summary**:

- To provide a high standard of patient centred pain management support and lifestyle coaching within the pain management service.
- To increase patients’ confidence and engagement in lifestyle change.
- To use recognised coaching techniques to help patients achieve their agreed goals.
- To help patients become more autonomous in their self-management strategies.
- To offer support to those who demonstrate low activity levels.
- To work with patients using reflection and planning techniques to support positive change.

**Clinical responsibilities**:

- Health and wellbeing coaches (HWBCs) will predominately use health coaching skills to support people to develop the knowledge, skills, and confidence to become active participants in their care so that they can reach their own health and wellbeing goals.
- To provide access to self-management education, peer support and social prescribing.
- To support people to self-identify existing issues and encourage proactive prevention of new and existing illnesses.
- To use effective communication and negotiation skills and support personal choice and positive risk taking.
- To work alongside physiotherapy and other MDT colleagues to coach and motivate individuals through multiple sessions, supporting them to identify their needs, set goals, and help them to implement their personalised health and care plan.
- To support self-management for individuals referred into the service.
- To use shared decision-making between with individuals referred into the service.

**Responsibility for patients**:

- To communicate effectively with patients to maximise rehabilitation potential and ensure their understanding of their condition, which will frequently require verbal communication skills including:

- Motivation
- Persuasion
- Explanation of complex issues
- To demonstrate flexibility in communication and interpersonal skills to overcome barriers to communication such as:

- dysfunctional illness beliefs
- altered perception and cognition
- language and cultural barriers
- long-term illness/dysfunction and/or uncertain prognosis.
- To communicate complex patient related information effectively, to ensure collaborative working with the multi-disciplinary team members across health and social care sectors ensuring the delivery of a co-ordinated multidisciplinary service.
