Assistant Practitioner for Frailty - are you an experienced and motivated practitioner seeking a new challenge, who enjoys working within a multi-disciplinary team and wishes to develop their clinical skills ?

A vacancy is available for the position of AP for Frailty at Warrington and Halton Hospital.

The post-holder would be responsible for supporting the MDT across the acute frailty pathway. This role requires excellent organisational and interpersonal skills, alongside sound clinical skills, relevant experience and the ability to work at pace.

The acute frailty pathway supports people living with frailty to self-care and maintain what matters to them most. We provide a truly integrated pathway, collaborating with our partners in primary care and local authority to improve patient care and communication across the whole system.

We use skill mix and role development to enhance job content and quality at all levels of the department. This is an ideal opportunity for an AP who is looking to enhance their clinical skills, collaborative working relationships, and to support more junior team members to develop and enhance their knowledge around caring for those people living with frailty. The frailty pathway is supported by an established frailty MDT and robust management team.

**The role includes**:

- Managing a caseload of patients and the delivery of therapeutic interventions under the supervision of qualified therapists and nurses.
- Working collaboratively as part of the MDT to deliver personalised interventions for patients across various settings.
- Formulating basic assessments within guidelines
- Screening of patients and adjustment of individualised treatment programmes.
- The role includes working flexibly within a hospital and a community setting, in accordance with the requirements of the Frailty pathway.
