Team for Older People - Social Care Assessor

Grade 07 Salary £29,269 -£ 30,825

The Team for Older People is looking to appoint a highly motivated Social Care Assessor.

You will work with individuals so that they are enabled to identify their own personal outcomes. Your work will focus on the individual and their support networks including families to promote independence and ensure the individual has a stronger voice in decision making and more control in their lives.

The Social Services and Wellbeing (Wales) Act (2014) and Mental Capacity Act 2005 provide legislative frameworks to ensure that the team complies with statutory duties and that individuals are supported to achieve positive outcomes.

To be the right personfor this role, you will have excellent communication skills, proven experience of working as part of a team, previous experience of working with vulnerable adults and the ability to respond appropriately to the needs of older people whilst working within statutory guidelines, policies and standards. The Team for Older People works closely with partner agencies to support and promote safe and timely hospital discharges, so experience of multi-disciplinary working would be an advantage.
