Guardian Angel Carers are now looking to appoint a care coordinator to join our fantastic team. We are growing as a business and as a result now have a great opportunity with full training, somebody who is passionate about care who wishes to progress with a genuinely caring company.

**MAIN DUTIES**
- Organise Care-Angels rotas to minimise travel between clients.
- Respond efficiently to day-to-day changes in clients care needs and make changes where required. Monitor all relevant aspects of the care management system.
- Ensure that all client visits are allocated and reflect clients service orders.
- Ensure all re-assessments are completed in a timely manner and records are updated accordingly.
- Undertake on call duties as per the office colleague rota.
