Exciting opportunity for enthusiastic and highly motivated individual to join our Gannel Dementia and Older People's Mental Health Team based at Trevissome Park, Truro.

We are a friendly and supportive team and strive to be an effective service, seeking to deliver high quality assessment and treatment for our patients.

We offer assessment and treatment for patients with a dementia who are experiencing Behavioural and Psychological Symptoms of Dementia, and also for patients over 75 with a severe functional mental health disorder.

We are looking for an individual who has a core professional qualification and registration in either mental health nursing, social work or occupational therapy to join our community service line. We are looking for an individual who has experience in working independently in a mental health setting.

As a DOPMH team member you must have a positive outlook and sense of humour to accept the challenges of working in a busy team that extends to seaside towns and rural locations. You will need to have knowledge of functional and organic mental health problems, the impact these may have on service users and their treatment. You will need to have knowledge of the CPA process, legislation around mental health & The Mental Health Act.

We need a person who can demonstrate passion and commitment to mental health issues have an approach to excellent client centred care along with working flexibly as part of a team, to deliver innovative health and social care.
- To be responsible for the assessment of care needs and clinical management of a defined caseload within the Cornwall Partnership NHS Foundation Trust (CFT) Mental Health Service.
- To carry continuing responsibility for the development, implementation and evaluation of programs of care, utilising the care co-ordination system.
- To develop the interface between primary and secondary care services so the patients/carers are offered focused and co-ordinated packages of care that are appropriate to their needs.

Approximately 568,000 people live here. A third of people who live in Cornwall are supported by acute hospital services in Devon. As a result, we also work closely with our partners in Devon. In the summer, and during other holidays lots of people choose to visit the area. This increases the numbers of people who use our services.
