**An exciting opportunity has arisen to really make a difference to the lives of older and vulnerable people.**
Age UK Thanet are seeking a Community Navigator to work as part of the ‘Connect Well East Kent Programme’ Age UK Thanet is working in a new consortium called ‘East Kent Strategic Partnership’ alongside Social Enterprise Kent, Age Well East Kent, (Age UK) and Carers’ Support across Canterbury, Dover and Thanet, to deliver a 4-year contract.
The Community Navigator will arrange and undertake visits to older and vulnerable adults with complex needs who are living in their own homes. They will work to ensure each older person is supported to live the life they wish to the best of their abilities, whilst keeping their lifestyle goals at the forefront of their care planning.
The Community Navigator will also be attending meetings with Local Care Teams and will be working with local statutory / voluntary sector agencies and other professionals to identify those who would benefit from the service.
Please note: A driving license is a must and a willingness to work occasional weekends / evenings would be essential for this role.
