**Job Title**: Family Group Conference & Lifelong Links Co-Ordinator x4.5

**Location**: Base Location Farnham House, Stevenage with the requirement to travel across Hertfordshire.

**Starting Salary**: £35,745 progressing to £39,186 pa

**Hours**: Full time and part time hours considered

**Contract**: Fixed Term until/Secondment until 31/03/2025

An exciting opportunity has arisen to help deliver a DfE funded project supporting Hertfordshire’s Care Leavers to help find, reconnect, and build supportive relationships with extended family and new networks.

Lifelong Links aims to ensure that children in care and care leavers have a positive support network around them to help them during their time in care and in adulthood.

We are looking for a positive and dynamic individuals to be a key driving force in engaging care leavers and their extended networks through activities and support groups, which will be vital to getting the Lifelong Links project up and running for Care Leavers.

The role of a the “Lifelong Links Co-ordinator” is to set up and run a range of engaging activities and support groups for Care Leavers to help bring them together on a regular basis, with the aim of building positive relationships and promoting “Lifelong Links”.

Other exciting elements of this role will see you actively managing a small case load of individual young people using the Lifelong Links model, to help them find out who is important to them, from their past and present, and work out how new and supportive relationships can be created for the future.

A key aspect of the role is providing on-going support to Care Leavers, through the Lifelong Links process by undertaking direct work using a variety of tools and techniques (genograms & timelines), to identify and connect their network. Working with young people to bring everybody together at their Lifelong Links Family Group Conference where a plan of ongoing support will be created and implemented.

**Essential Criteria**:

- Insight and ability to help Care Leavers manage and grow positive relationships, this may require you to support and mediate between individuals in the network.
- An understanding and ability to manage groups and meetings.
- Ability to be flexible in your working hours, evenings and weekends may be required to meet the needs of this role.

**Desirable Criteria**:

- A social work qualification or professional equivalent qualification. You must be registered with SW England if you are Social Worker.
- A knowledge of Family Group Conferencing and Lifelong Links would be helpful and if trained either or both this would be very advantageous.
