Yn y lle cyntaf i’r unigolion efo ananableddau dysgu yn eu man gwaith dyddiol, ac yna i’r Rheolwraig/ŵr Prosiect perthnasol, ac yn y pen draw i’r Brif Weithredwraig a Bwrdd y Cwmni. Bydd yn ofynnol i deilydd y swydd gyflawni ystod o ddyletswyddau yn unol â chyfarwyddyd rheolwyr man gwaith yr unigolion efo anableddau dysgu sy’n derbyn y gefnogaeth.
Bydd gofyn i deilydd y swydd gymryd cyfrifoldeb llawn dros yr unigolion efo anableddau dysgu yn y man gwaith, gan gynnwys mynychu unrhyw apwyntiadau fydd gan yr unigolion.

**DISGRIFIAD SWYDD - GWEITHIWR/WRAIG CEFNOGOL (DYDD/PRESWYL)**
**Cyflog - £18,525 y flwyddyn pro rata (£9.50 yr awr) i weithio 8dydd Llun - dydd Sul (yn unol â rota) yn cynyddu i £19,305 y flwyddyn pro rata (£9.90 yr awr) yn dilyn cyfnod prawf llwyddiannus a cyflawni Hyfforddiant ar gyfer cofrestru gyda GCC**
**Lwfans Cysgu i Mewn - £80.75**
**Lleoliad: Yn ddibynnol ar safle gwaith yr unigolyn gyda anableddau dysgu sydd yn derbyn y gefnogaeth gwasanaeth dydd/preswyl. Gall fod yn ardal Waunfawr, Caernarfon neu Penygroes.**
1. Cefnogi’r unigolyn gyda ananableddau dysgu yn eu man gwaithac adref, i ddatblygu eu sgiliau. Bydd angen annog yr unigolion i fynychu gweithgareddau iechyd a llesiant i gyd fynd a’r cynlluniau gofal.
2. Sicrhau bod y rhaglen gweithgareddau iechyd a lles yn cael eu gweithredu yn gyson yn unol â gofynion yr unigolion gyda anableddau dysgu.
3. Sicrhau bod y Rheolwraig/ŵr Prosiect yn cael gwybodaeth am unrhyw anhawsterau gall godi yn y cartref neu’r lleoliad/au gwaith.
4. Meithrin perthynas dda gyda’r asiantaethau sy’n cynnig gwasanaeth ar gymuned yn ehangach.
5. Mynychu unrhyw apwyntiadau meddygol fydd gan yr unigolion gyda anableddau dysgu.
6. Mynychu unrhyw gyfarfodydd adolygu ar gyfer yr unigolion gyda anableddau dysgu.
7. Yn unol â’r angen sicrhau bod trefn cofnodi a dosbarthu meddyginiaeth i’r unigolion gyda anableddau dysgu yn cael ei gyflawni’n briodol.
